***If your student will be absent, please call the school by 8:00 am to let us know why.  If your student is out sick, the health department requires the nurse to report two symptoms.  If going to the doctor or dentist, a doctor’s note is required for the absence to be excused.   Notes can be brought to the office or faxed to the office.  The fax # is 563-445-5963.

Please remember that all medicines (prescriptions or over the counter) require signed documentation in the nurse’s office and must be stored in the nurse’s office.  This is for the safety of our students.  Prescriptions require the doctor and parent signatures and must be in the original prescription container.  Over the counter medications, such as cough syrup, cough drops, pain relievers, etc., require a parent signature and must be in the original, unopened container.  If you have any questions or need a form, please contact school nurse Kristen Dose at the number above or email at dosekr@davenportschools.org or Health Para Rebecca Amissah at amissahr@davenportschools.org

Regarding head lice:  We encourage parents to screen their children weekly.  Please call the school to report if your child has lice. View the DCSD head lice procedure.
